If you are late in completing your requirements OR have just recently submitted them, do not panic. We are processing forms as quickly as possible. Student should watch for a new secure message on MyJMUChart with the status of their records. Processing may take several weeks. Holds will not go on until February and students will be notified if a hold is placed on their account.

As a new student at JMU, you are required by Virginia law to submit specific information about immunizations and complete a TB screening.  JMU does NOT require a physical exam, but you must comply with the immunization requirement and TB screening. For students starting in January, the due date will be the first week in February. Failure to complete these steps will result in a nonrefundable fine and you will not be able to make changes to your class schedule or enroll in classes for future semesters.

View this video for step-by-step instructions. Please refer to The One Book for dates and deadlines. It is mandatory that you complete the following steps and that they are confirmed by UHC before you attend classes.

  1. Submit your official immunization record from your MD or from another school or university. Click on the Immunization Form if you do not have access to an immunization record. Print the form and have it completed and signed by a health care professional. If a different immunization form is used by your health care provider or you have a completed one from another college or university, it will be accepted in place of the JMU form. A signature is required only if the JMU form is used.
  2. Log in to your MyJMUChart account to upload a scanned copy of the immunization form, as well as a copy of your health insurance card if you have one:
    a) Upload the PDF documents into MyJMUChart – it’s just like attaching a document to an email
    b) Remember, ALL UPLOADED FORMS MUST BE IN PDF FORMAT
  3. While in your MyJMUChart account, complete the required TB Assessment and Health History under the “forms” tab. You will be notified if a TB skin test is required.
  4. Under the Insurance tab on MyJMUChart, upload a copy of the front and back of your insurance card. Please also manually enter the information. The Policy number requested is the ID number (the group number is not necessary). The information requested such as name, birthday, etc. is for the carrier of the insurance, which is the name that appears on the card (probably a parent, guardian or yourself).

Continue checking your JMU dukes email and secure messages on MyJMUChart for confirmation that your record is complete and there is nothing further needed. These requirements are not complete until you receive that confirmation. PLEASE check your JMU email from communication from the Health Center. Forwarding your JMU email to another email address can delay receipt of vital information.

For students who are starting in January, the due date will be the first week in February. Failure to complete the above steps will result in a fine and you will be prohibited from making additions or changes to your class schedule, and will not be allowed to register for any classes in future semesters.
