Fill out the form 

The MAC address field is required.  To find it (on a Mac computer): 

  1. Click on the Wifi Icon in the upper right corner of your screen
  2. Click Open Network Preferences
  3. Click Advanced in the Network window pop-up
  4. The Wi-Fi Address at the bottom of the screen is the MAC address

On a Windows PC

  1. Right-click the Wifi icon on the bottom right
  2. Click Open Network and Internet Settings
  3. Click View Your Network Properties
  4. Scroll down to the Wifi section (there is a section for every connection so make sure you are looking at the proper one)
  5. The MAC address is listed under Physical Address (MAC)
