Undergraduate Courses
| Use this form… | If you want to… |
| Undergraduate Course New (form preview) | Propose a new undergraduate course. If the course will be dual-level, use the Graduate Course New form. |
| Undergraduate Course Modify (form preview) | Modify an existing undergraduate or General Education course. |
| Undergraduate Course Delete (form preview) | Delete an existing undergraduate or General Education course. Note: If the course is part of a program, you will also need to complete a program action request. |

| Graduate Course New (form preview) | Propose a new graduate course or propose a new dual-level course. |
| Graduate Course Modify (form preview) | Modify an existing graduate or dual-level course. If the course is undergraduate, use the Undergraduate Modify Course form. |
| Graduate Course Delete (form preview) | Delete an existing graduate or dual-level course. Note: If the course is part of a program, you will also need to complete a program action request. |

Use this form to create a new undergraduate course that will be part of General Education. |
| Undergraduate Course Modify (form preview) | Modify a course that is currently part of General Education. This proposal will seek approval from both the offering academic unit and the appropriate General Education cluster. |
| Undergraduate Course Delete (form preview) | Delete a course that is currently part of General Education. This proposal will seek approval from both the offering academic unit and the appropriate GenEd cluster. |

New Majors and Programs
Proposers must contact Paula Maxwell, Associate Vice Provost for Faculty and Curriculum, to complete the preview process for new majors, minors, certificates or other programs that require resources.
