Our CHW Program is dedicated to improving the health and well-being of underserved communities by empowering local residents to become trusted health advocates. Community Health Workers (CHWs) are essential frontline public health workers who serve as liaisons between the community and healthcare services. By building strong, trusting relationships, CHWs help individuals navigate complex health systems, access resources, and adopt healthier lifestyles.
What Does the CHW Program Do?
- Health Education & Promotion: CHWs provide culturally relevant health education to individuals and groups, addressing topics such as chronic disease management, nutrition, and preventive care.
- Connecting to Resources: CHWs help community members access vital services, including healthcare, social support, and financial assistance.
- Advocacy & Support: CHWs advocate on behalf of their clients, ensuring their needs are understood and met by health and social service providers.
- Bridging Cultural Gaps: CHWs work to bridge the gap between healthcare providers and the community, ensuring care is delivered in ways that respect cultural values and traditions.

Mission
To be the key resource for training CHWs in English and Spanish in the Central Shenandoah Valley, including hosting CHW Community of Practice, and serving as a resource to employers for developing supervisory and structural models for embedding CHWs in their work to improve individual and population health.
