Series Description: The Chronic Disease Self-Management Education program is an evidence-based program that was developed and researched by Stanford University for people with chronic conditions such as diabetes, arthritis and hypertension.
Session 1: Introduction to Workshop; Group Introductions; Mind-Body Connections/Distraction; Getting a Good Night’s Sleep; Introduction to Action Plans
Session 2: Feedback/Problem Solving; Dealing with Difficult Emotions; Introduction to Physical Activity and Exercise; Preventing Falls and Improving Balance; Making an Action Plan
Session 3: Feedback; Making Decisions; Pain and Fatigue Management; Endurance Exercise; Relaxation/Body Scan; Making an Action Plan
Session 4: Feedback; Better Breathing; Healthy Eating; Communication Skills; Problem-Solving; Making an Action Plan
Session 5: Making Healthy Food Choices; Medication Usage; Making Informed Treatment Decisions; Depression Management; Positive Thinking; Making an Action Plan
Session 6: Feedback and Problem Solving; Working with Your Health Care Professional and Health Care Organization; Weight Management; Looking Back and Planning for the Future
