 Student HEALTH Insurance

Student Health Insurance is available through Academic HealthPlans

The JMU Health Center is pleased to announce that Academic HealthPlans has agreed to provide JMU students with Health Insurance and Dental Insurance. Dental Insurance is a separate policy with Delta Dental and is available for purchase through September 15^th each year. Academic HealthPlans is underwritten by ACE American Insurance Company and all claims for reimbursement are managed by Klais & Company, Inc.

Who can enroll?

• Undergraduate students enrolled in six (6) or more credit hours
• Undergraduate students enrolled during the summer in three (3) or more credit hours
• Graduate students enrolled in three (3) or more credit hours

How do I enroll?

Complete an enrollment form on line at Academic Health or print out the enrollment form and submit by mail. Your premium payment must be received at the time you enroll in the Plan.

When do I enroll?

Enrolled students can complete the application for insurance coverage at any time for the various coverage periods.

When is the effective date of coverage?

The effective date of coverage begins:

• The day after the date the premium is received by the Company or its authorized representative
• Or on the effective date of the coverage period, whichever is later

Claims Reimbursement

Mail any medical bills to the to Klais & Company, Inc for reimbursement.  Make sure the following information is included with the bills (some of the information may be printed on the bills, others you may need to provide):

• Name
• Date of Birth

• Name
• Address
• Social Security Number
• Name of school student attends

File claims within 30 days of Injury or first treatment for a Sickness or as soon as reasonably possible. Bills should be received by the Company within 90 days of service. Bills submitted after one year will not be considered for payment except in the absence of legal capacity.
