Immunization Requirements

Each semester the University Health Center requires new incoming students to complete a Health Evaluation/Immunization form. Compliance with and completion of this form is a Commonwealth of Virginia and University requirement for all students. Students will not be allowed to register for a second semester without full compliance.

Students are strongly encouraged to meet these requirements at home and return the completed form by the health record deadline.

Immunizations can be obtained through local Health Departments in Virginia or through private health care providers. Students may also obtain immunizations through the University Health Center.
