THE UNIVERSITY HEALTH CENTER REQUIRES A COMPLETED HEALTH RECORD FORM ON FILE, PRIOR TO YOUR ENROLLMENT AT JMU.
2008 Health Record due dates:
January 31st, 2008 for Spring Semester incoming students.
June 2nd, 2008 for Fall Semester incoming students.
DO NOT FAX THE COMPLETED HEALTH RECORD FORM.
