Dear JMU student,

**REMINDER-Final Notice**

According to our records, we do not have your documentation of having received the full COVID-19 vaccination or a signed Student Assumption of Risk Waiver. Please electronically upload one of these right away. These will be added to your MyJMUChart and you will be marked as having completed this step.

If we do not receive your record or waiver via the electronic submission by 9 a.m. on Monday, Sept. 6 your JACard will be deactivated.

If you are receiving this email by mistake, please re-upload your documentation so that we can correct our records.
