Appointments

• The Health Center operates on an appointment-based system. Please call 568.6178 to schedule an appointment.
• If this is your first visit, please bring all of your medication bottles to show the nurse.

Things to remember

• If you are driving allow extra time to find parking. Parking is very limited, we suggest that you walk or take the bus.
• If you are 5 minutes late your appointment will be re-scheduled.
• If you are unable to keep your appointment, please give us at least an hour's notice, so that the slot can be used for another student.
• A "no show" fee will be charged when a student has a scheduled appointment and does "not show" or call to cancel or reschedule the appointment.
• No show fees are $10.00 for a 15 minute appointment and $15.00 for a 30 minute appointment.

When you arrive

• You will need to provide your JAC card to receive service.
• If you park in a "Reserved UHC Patients Only" parking space, we will need your license plate number.
• All payments are due at the time of service.
• Forms of payment accepted are cash, check, FLEX or **MC & Visa credit cards

**University policy requires that the name on the credit card used for payment must be the same as the person signing the credit card payment.
