     Birth Certificate:
     If your child was born in VA, contact the Office of Vital Records, VA Dept of Health 
          to order a birth certificate.
     If your child was born in another state within the US, consult the
          National Center for Health Statistics.

 




Families of children who are currently enrolled may review and update forms at any time by contacting their child's teacher. It is particularly important that health care information and emergency contact numbers remain current.  

Families of children on the waiting list for future enrollment should contact Mrs. Danelle Cross, YCP Secretary, at 568-6292 or crossdr@jmu.edu to update information on the registration form.
