Reminder to upload vaccination data into MyJMUChart

Faculty, Staff and Graduate Assistants,

To inform various stakeholders of JMU’s rate of COVID-19 vaccinations received by members of the JMU community, we ask that you upload your completed COVID vaccination record to the University Health Center (UHC) secure patient portal on MyJMUChart. Remember to wait until you are fully vaccinated to follow these steps:

STEP 1: On the Pre Sign-In Notification page, click Proceed.

STEP 2: On the SSL VPN Access Service page, enter your eID and password. For the Secondary password from Duo, type either push or sms1. Click Sign In, then Accept to accept the prompt from Duo and proceed.

STEP 3: On the PulseSecure page, under Web Bookmarks, click MyJMUChart.

STEP 4: At the top of the page, log in to MyJMUChart by entering your JMU eID and password. Click Login.

STEP 5: In the gray bar at the top of the page, click Upload. Under Choose document you are uploading, select COVID-19 Vaccine Documentation from the dropdown menu, then click Select File.

STEP 6: Browse the files on your computer and select your COVID vaccination record, then click Open.

STEP 7: Click Upload.

*Faculty and staff who have older eID’s may experience difficulty with the electronic upload and can use other ways to submit their documentation.

You may also submit your documentation by:

  • Emailing healthctr@jmu.edu
  • Faxing to 540-568-6176
  • Placing a copy in the immunization drop box outside of the UHC second floor entrance

All documentation submitted must be legible, include your name and birthdate, and a copy of the documentation proving that the vaccine was given.

Employees are not required to take the vaccine. Choosing to be vaccinated is your personal and voluntary choice.
