Immunization requirements confusing?

It may take some time to round up your immunization dates and get in to see a health care provider, so get on it!

Incoming first-year students, did you receive an email from the University Health Center about your immunizations not being in compliance?

The immunization form is a two-step process that is required by the state.

  1. Log-in to the secure, online portal, MyJMUHealth, and follow the instructions to enter your immunization dates, insurance information, and brief medical history.
  2. Download, print, complete immunization form from MyJMUHealth, have it signed by a health care provider, and mail it to the Health Center by August 1.

It may take some time to round up your immunization dates and get in to see a health care provider, so get on it!

Faxed copies of the immunization form will not be accepted. Failure to complete the two step process by August 1 will result in an academic hold and $50 fee on your student account.
