You are receiving this message because your name appears on a list of JMU employees who have not provided a COVID-19 vaccination record to the University Health Center (UHC). This means you are either unvaccinated or you have not yet submitted your vaccination record.

By Tuesday, August 10 at 5 p.m., please do the following:

  1. If you are vaccinated or have started your vaccination process, submit your vaccination record to the UHC (see below for instructions).
  2. If you feel you have received this email in error, reply to this correspondence to indicate the reason.

To date approximately 70% of faculty and staff have submitted a vaccination record. While we know that many others have been vaccinated or have begun the vaccination process, the low percentage of vaccinated employees is concerning. There are widespread reports of increased COVID-19 cases due to the Delta variant, and other rapidly changing developments are in the news. JMU is considering stricter measures to maintain the safety of the JMU community. Therefore, it is more important than ever that you submit your vaccination record to the UHC.

HOW TO SUBMIT YOUR VACCINATION RECORD

The easiest and quickest way to send your COVID vaccination record is by email. To do this, complete the following steps:

  1. Scan or photograph both sides of your COVID vaccination record and draft an email to healthctr@jmu.edu
  2. Attach the scanned or photographed images to the email
  3. Include your name and birthdate in the body of the email
  4. Send the email

OTHER OPTIONS YOU MAY USE TO PROVIDE YOUR VACCINATION RECORD

You may also submit your documentation by:

  • Placing a copy in the immunization drop box outside of the UHC second floor entrance
  • Uploading images of your vaccination record to MyJMUChart

All documentation submitted must be legible and must include your name, birthdate and a copy of your vaccination record. Please visit the JMU Employee Vaccine FAQ for more information.
