CARE APPLICATION

In order to provide information about your previous experiences and skills, please respond to the following questions.  All information will be reviewed ONLY by CARE Core and will be used only for the purpose of making recommendations about training.  Previous experiences or training is not required to become a volunteer.

 

Please type all responses on a separate sheet and include your name, address, phone number, email address, year in school, and current credit hours at the top of the page.

 

Part One

 

1.     What other time commitments, organizations, or activities are you involved in?

2.     Please describe any previous formal training or course work in peer advising or counseling, basic counseling skills, conflict resolution, crisis intervention, or communication skills.

3.     Please describe any previous experience providing formal or informal peer counseling or advising.  Please include the nature of the work, the setting, and the length of service.

4.     Have you ever been convicted of a criminal offense?  If so, please explain.

5.     Have you ever been found guilty of a judicial offense at JMU?  If so, please explain.

6.     Have you ever been involved directly or indirectly in a sexual assault situation?  If so, please discuss how you think this may influence your role as a volunteer.

 

Part Two

 

On a separate page, please include a personal statement describing your interest in becoming a CARE helpline volunteer and/or any experiences or knowledge relevant to your effectiveness on the helpline.  Please make this no longer than two pages.

Please list two references (not family) that we may contact if needed.
