JMU Office of Financial Aid & Scholarships

Scholarships from Institutional Funds

Use the Departmental Scholarship Request Form and Award/Scholarship Disclosure Questionnaire to notify the Office of Financial Aid and Scholarships about awards you intend to pay from institutional funds, including your department’s budget. The disclosure questionnaire will ensure proper classification of awards. If the award conforms to the definition of scholarship, departments may submit a questionnaire and scholarship request form to the financial aid office for processing. Submit one scholarship request form and one disclosure questionnaire per student. For awards that fail to meet the definition of scholarship, your students must receive payment from the Office of Payroll Services.
