WHAT TO DO IN CASE OF ACCIDENT

1. Medical Attention First: Call nearest rescue squad and Virginia State Police. If outside of Virginia contact State Police or Sheriff, of City Police with jurisdiction.

4. Information to Obtain
   
   
   1. Names of all parties involved
      
      
   2. Names of witnesses
      
      
   3. Name of investigating police officers
      
      
   4. Name of Rescue Squad Supervisor
      
      
   5. Name and Policy Number of Insurance Company of those involved.
      
      
   6. Date, Time, Location of accident
      
      
   7. Numbers for all Incident Reports filled out by Police Officers
      
      
   8. If Complaint Sheet is filled out, ask officer or Rescue Squad personnel for copy of that report.
      
      
   9. If anyone is taken to medical care facility:
      
      
      
      1. Get Name of Facility
         
         
      2. Examining Physician
         
         
      3. Nurse Practitioners

   INFORMATION SHOULD NOT BE IMPARTED TO OTHERS EXCEPT INVESTIGATING POLICE OFFICERS
