Add or Remove Vehicle Request Form

Instructions:

Please complete the required fields in section 1 and provide information for the vehicle(s) that you would like to add or remove in Section 2. Once you complete and submit this online form, Parking Services will make the requested changes to your record. Rental or loaner vehicles cannot be added to employee hangtags. If you have a rental or loaner vehicle, bring the registration or rental agreement for the loaner vehicle to Parking Services to obtain a temporary permit.

Registered owner is responsible for vehicle registration, citations and misuse of hang tag.
