Please make sure all required signatures are on the form before submitting it to Debbie Juhasz (firstname.lastname@example.org) at The Graduate School. Students meeting degree requirements in summer but walking in May should mark their graduation application accordingly.
Students must have unconditional status, having met any requirements accompanying conditional acceptance and submitted the Graduate Status Change Request form.
Students must be registered for a class (not GRAD 597: Continuance).
Ensure that all course substitutions/waivers and credit transfers have been processed by Debbie Juhasz at The Graduate School to explain discrepencies between your program of study and program requirements.
Review the Degree Requirements Checklist from the Graduate School, with additional items, such as purchasing graduation attire.
Debbie Juhasz can accept faxed and/or emailed documents. If you prefer to fax or email paperwork, there’s no need to follow up with the original form.
