In order to assist you with the completion of the Administrative & Professional Faculty Job Description and Performance Evaluation, the document has been divided into separate Word documents. Please click on each link to complete the section. It will be necessary to save these files to your computer.

Current A&P Faculty Positions

  • Each time the Job Description changes, update Section 1, sign and send to Human Resources.
  • Sections 2 and 3 of the A&P Faculty Job Description and Performance Evaluation form should be completed between June 1 and August 31 of each year and returned to the Human Resources department no later than the last working day of September.

New A&P Faculty Positions

  • Fill out Section 1 and send to Human Resources.
  • Supervisor and employee may develop objectives in Section 2 for the upcoming performance cycle.
  • Sections 2 and 3 of the A&P Faculty Job Description and Performance Evaluation form should be completed between June 1 and August 31 of each year and returned to the Human Resources department no later than the last working day of September.

Section 1: Job Description and Key Responsibilities
This section describes the applicable competencies for all A&P Faculty Members. This section must be completed for all A&P Faculty positions. The Position Description is submitted to HR either when the position is established or when duties are updated. Download Section

Section 2: Objectives for this Performance Cycle
Objectives for this Performance Cycle: This section outlines the employee's key goals and/or objectives for the performance cycle. Special consideration should be given for the comments made in Section 3C of the performance evaluation that pertain to focusing the employee's efforts toward improvement. Download Section

Alternately, supervisors may choose to use a narrative document instead of the Administrative & Professional Faculty Job Description & Performance Evaluation form if the Job Description (Section 1 of the form) has been filed with Human Resources. In addition, all performance evaluations written in narrative form must include objectives for the performance cycle and an evaluation of the employee’s performance along with any needs or goals for professional development. Please refer to Policy 1307 Section 6 for more details.
