Summer Fee/Office Visit service fee

SUMMER FEE SCHEDULE or Office visit service fee based on eligiblity status:
  • $10.00 per 15 minute appointment; $15.00 per 30 minute appointment
  • Payment must be made by cash, check, FLEX or **MasterCard/VISA PRIOR to being seen for their scheduled appointment.
  • Any lab fees incurred will be in addition to the $10/$15 fee for service payment.

    **University policy requires that the name on the credit card used for payment must be the same as the person signing the credit card payment.
