Immunizations are available by appointment only from 8:00 am -- 5:00 pm, Monday through Friday. Please call 540.568.6178 for an appointment.
Each semester the University Health Center requires new incoming students to complete a Health Evaluation/Immunization form (Health Record form). Compliance with and completion of this form is a Commonwealth of Virginia and University requirement for all students. Students will not be allowed to register for a second semester without full compliance.
Students are strongly encouraged to meet these requirements at home and return the completed form by the health record deadline.
Immunizations can be obtained through local Health Departments in Virginia or through private health care providers. Students may also obtain immunizations through the University Health Center. You will need to call 540.568.6178 to make an appointment for any needed immunizations.
Minimal fees are charged for vaccines given at the University Health Center.
**The Hepatitis B Vaccine series is a series of three vaccines. If you have started the vaccine injections and are waiting to complete the series, again please sign the Hepatitis B waiver, submit your health record and send verification of the series when you have received the injections.
The Hepatitis B vaccine series (3 shots) is mandatory for all IN-COMING STUDENTS, beginning in the Fall of 2006.
Vaccinations not listed are NOT available at UHC. If necessary, you will be referred to the Harrisonburg-Rockingham County Health Department at 540.574.5100.

Institutions where your immunization records might be found usually have rules for how long they are kept, so very old records may have been destroyed. Regardless of the type of vaccine documentation you have, you must submit the JMU Health Record signed by a healthcare provider. Under certain circumstances, other documentation may be accepted, but providing such documentation may delay your ability to register for classes.
