Summer Session Eligibility

SUMMER SESSION 2009

May, June, July 2009

During the week of May 11th through 15th all appointments will be charged the summer eligibility office fee for services.

Beginning Monday, May 11th through August 7th any student NOT enrolled in summer classes will need to pay an office fee for any appointment.

May, June, July Exceptions to the office fee for services:

• Allergy/Immunization appointments
• Women's Health Program appointments

'No Fee' eligibility

Last day of 'NO FEE' eligibility for summer session students is Friday August 7th.

During the weeks of August 10th through 14th, ALL JMU students who are enrolled for fall session will be charged an appointment/eligibility office fee for services at UHC.

August exceptions to the office fee

• Allergy/Immunization/TB appointments
• Women's Health Program follow-up students
• Any student required to be on campus early, for example: RA's, FROGS, band members and athletes.

Appointments without a fee for all eligible enrolled JMU students will begin on August 17, 2009

Fees

Summer fee schedule or Office visit service fee based on eligiblity status:
$15 Fee per office visit

Payment must be made by cash, check, FLEX or **MasterCard/VISA PRIOR to being seen for their scheduled appointment.

Any lab fees incurred will be in addition to the $15 fee for service payment.

**University policy requires that the name on the credit card used for payment must be the same as the person signing the credit card payment.
