Summer Session Eligibility

SUMMER SESSION 2011

May, June, July 2011

• During the week of May 9th through 13th all appointments will be charged the eligibility office visit fee.

• Last day of eligibility for summer session students is Friday August 5th.

• From August 8th through 19th, all eligible students who are enrolled for fall session will be charged a $20.00 office visit fee

• 2011/2012 incoming freshman will not be eligible for Health Center services until August 23rd

• Beginning Monday, May 16th through August 5^th,  2011, any student not enrolled in summer classes will need to pay a $15.00 office visit fee (exception: Women’s Health follow-up appointments for birth control only)

• Beginning August 23^rd, all 2011/2012 students are eligible with no office visit fee applied

May, June, July Exceptions to the office fee for services:

• Allergy/Immunization appointments

'No Fee' eligibility

Last day of 'NO FEE' eligibility for summer session students is Friday August 5th.

During the weeks of August 8th through 19th, ALL JMU students who are enrolled for fall session will be charged an appointment/eligibility office fee for services at UHC.

August exceptions to the office fee

• Women's Health follow-up appointments for birth control only

• Any student required to be on campus early; for example: RA's, FROGS, band members and athletes

Appointments without a fee for all eligible enrolled JMU students will begin on August 23, 2011

Fees

Summer fee schedule or Office visit service fee based on eligiblity status:
$20 Fee per office visit

Payment must be made by cash, check, FLEX or **MasterCard/VISA PRIOR to being seen for their scheduled appointment.

Any lab fees incurred will be in addition to the $20 fee for service payment.

**University policy requires that the name on the credit card used for payment must be the same as the person signing the credit card payment.
