Submit your health form All students need to have a completed health form on file with University Health Center. There will be a hold on your account until the completed form is received. If you are new to JMU, please print out a copy from the Health Center web site. The form requests immunization records, family history information, and emergency contact information. Part of the form needs to be completed by a physician and may require a recent physical evaluation. Follow the directions on the form exactly and mail (don't fax) the form in as soon as possible. Students returning to JMU may want to check with the University Health Center to see if their previously submitted Health Form is still on file. Distance learning students who will not be coming to campus for classes should check with the Health Center to see if they need to complete a Health Form.
