The Madison Achievement Scholarships recognize a broad spectrum of accomplishment and potential. To notify the financial aid office regarding new and renewal awards based on your allocation, complete a scholarship nomination form and Award/Scholarship Disclosure Questionnaire. The disclosure questionnaire will ensure proper classification of awards according to the definition of scholarship. Submit one scholarship request form and one disclosure questionnaire per student. |
