A Sample HTML Form

The form below submits its collected data to the DoForm program residing on the web server. Then DoForm executes the script (.cfg file) you've specified in your HTML which in turn mails the result me, stores some of the data in a file, displays a customized message to the person submitting the file and even verifies that certain form fields have reasonable values.

After completing and submitting the form you can return to this page and view the results of the form submission to the DoForm script controlled by the SAMPFORM.CFG configuration file.

SAMPFORM.ORD
This is the Unix mail folder which contains the messages which have been sent to during the submission process.
SAMPFORM.LOG
This shows the default results format and lists all of the fields submitted to the script.
SAMPFORM.DAT
This shows the data being collected for analysis with a statistical package.
SAMPFORM.MKT
This file holds the address information for the fools who requested marketing hype.
