1. Complete the Non-Degree Seeking Student Application form online.
2. Submit online the completed form
and the $45 application fee to the College of Graduate and Outreach Programs.
**Most certificate programs
require suplementary materials. Please submit these materials as directed
by each program. Your application will not be considered complete without
these materials.
You will receive an official written notification
of acceptance/non-acceptance by mail from the College of Graduate and Outreach Programs.
If you are a GRADUATE degree seeking student and wish to add a certificate to your degree:
1. Complete the Request
to Add a Certificate Program form. This form must be signed by the
assigned graduate advisor and the certificate program coordinator.
2. Submit the signed
form to the College of Graduate and Outreach Programs.
If you are an UNDERGRADUATE degree seeking student and wish to add a certificate to your degree:
1. Complete the Change of
Major/Minor form available from the Registrar's
Office.
2. Submit the signed
form to the Registrar's Office.
