This printable form is required to grant/change access to the Human Resource,
Student Administration or Finance system. Only one copy of the form is required to
request access to one or more systems. A hard copy of the completed form should be
sent to the attention of the appropriate Data Managers listed at the top of the form.
How soon your access request is processed varies by system and request (most take

Also known as a Cost Benefit Analysis (CBA) or simply, a request. This
process is required to ensure appropriate, objective analysis of impacts
(both functional and technical) of a request. A CBA request is a users
first step in requesting either data analysis (a query, report, etc.), a
change to any component of an Information System at JMU, or a simple
request to determine if a system may already exist for a specific business
process. To submit a CBA request, follow the link above. You will be
directed to a secure site where you will be prompted to enter your emailid
and password. Once submitted, the CBA will automatically be sent to the
user services group within Information Systems How soon your analysis
