Alumni Group Forms

Alumni Volunteer Award (AVA) Nominations
Nominates alumni groups and volunteers for outstanding efforts. Only chapters who complete annual reports on time will be considered for AVAs. Submit a nomination.

Check Vouchers
Documents chapter spending. Must always be accompanied by itemized receipts. MUST be approved and signed by chapter president or president-elect to be reimbursed by the Office of Alumni Relations. Download the form.

Duke Dog Request
Alumni groups: would you like to request Duke Dog for your next event? Email Alan Maynard (maynaraj@jmu.edu) for more information. 

Event Summaries
Summarizes logistics and attendance of each event. Must be submitted to the Office of Alumni Relations to be reported in Viking. Submit the form.

Grant Application
The JMU Alumni Association is now offering at least 10 programming grants of up to $500 each for official alumni groups! These funds may provide the total funding for a group’s program or event, or partial funding to lower costs to alumni, or they may broaden the reach or impact of the program or event. Download the form.

New Group Request
States purpose and intentions of new alumni groups. Must be completed prior to starting chartering process. Submit a request.

Official Event Request Form
This form should be completed for any and all alumni events to allow the Office of Alumni Relations to publicize your events and track your alumni activity. This form should be submitted at the beginning of your event planning process. Submit a form.

Speaker Request Form
If you'd like to invite a JMU alumnus, administrator or faculty member to speak at an upcoming event, please complete this form and submit to the Office of Alumni Relations at least three months in advance, as some speakers require more advanced notice than others. Request a speaker.

Spirit Store Form
If your alumni group is in need of JMU spirit items, please complete this form and submit it to the Office of Alumni Relations. Items will be mailed to you within a week of receiving this form. Download the form.

Volunteer Sign up Form
Use this form at your alumni events to gather contact information for future volunteers, or to sign up as a volunteer yourself. Download the form.
