This form is only for students who have received an official admission decision and wish to discuss that decision with a counselor.
Inquiries will become part of the applicant's permanent record.
Transfer Applicants:
Under the regulations of the Family Educational Rights and Privacy Act (FERPA), we will only discuss decision inquiries with the applicant.
